Vibration and Stress

Many studies over the years have suggested that vibration, imposed on the human body through an external device, is an example of a harmful mechanical stress on the body, and can therefore have a significantly negative impact. In this case, as many individuals might think, the word stress is used to describe, as defined by Merriam -Webster, “a physical, chemical, or emotional factor that causes bodily or mental tension and may be a factor in disease causation.” However, this word has many definitions and for the vibration training and vibration therapy devotees out there, those who have experienced the euphoric feeling of 2400 involuntary muscle contractions in 60 seconds, stress is better defined as “a force exerted when one body or body part presses on, pulls on, pushes against, or tends to compress or twist another body or body part.” It is through the POSITIVE impact on the central nervous system that this type of mechanical stress keeps my training clients and physical therapy patients hungering for more.

The Effects of Vibration on the Central Nervous System

So…How is this form of stress responsible for the happiness some people experience as they laugh their way out the door from a WBV session; the “freedom of movement” that they speak about so often; the sudden burst of energy described by my clients in e-mails the day after using the Hypergravity platform; the ability my stiff arthritic patients have to “bounce” up the spiral staircase leading out of my building? On more than one occasion, I have even heard of enhanced psychic abilities …no lie! If their brains and spinal cords had a voice, I believe they would answer these questions with three words: Neurogenesis, Serotonin, and Endorphins. These three things, time and time again, have all been positively linked to exercise. Even better, they have all been linked through legitimate scientific research (and to think, here in NY, everyone wants rats to be exterminated!). Neurogenesis refers to the creation of new nerves, and occurs in an area of the brain known as the hippocampus, the same region where learning and memory occur. The process of neurogenesis, according to researchers, has to do with exercise stress-induced formation of a brain protein (known as BDNF) whose job it is to not only create new neurons, but to protect the existing neurons and enhance communication between them (otherwise referred to as synaptic plasticity). It is through synaptic plasticity that all learning and memory can occur in the brain. This includes muscle memory and motor learning, two important concepts that are clearly enhanced through WBV. Now, as BDNF levels rise in the brain, so do the levels of another brain protein referred to as SERT. I can happily say that this protein is responsible for increasing serotonin levels in the brain. For those who are unfamiliar with serotonin, it is a type of chemical, called a neurotransmitter, that is responsible for mood elevation, feelings of ecstasy, increased sexual desire and function, and improved sleep. What more can you ask for in life, I ask?–How about a “runner’s high?” “Runner’s high” is a term used to describe what is otherwise known as an endorphin rush. Endorphins are chemicals released by the pituitary gland in response to stress (there’s that word again) or pain. In general, it takes up to 30 minutes to release endorphins through traditional exercise, but with more intense “hyper”stimulation of the central nervous system, such as that with vibration training, this release occurs much more quickly. Once released, endorphins do everything from blocking pain to creating a sense of euphoria or exhilaration. Interestingly enough, endorphins bind to the same receptors in the brain as heroin and morphine (anyone ever heard the term “addicted” from their clients?) and by blocking pain, it is theorized that this is what allows for the continued presence of serotonin in the brain. Although more research is still required, there is a clear potential connection between whole body vibration, increased central nervous system stress, relief from pain, mood elevation and exhilaration, and, most importantly, neurological brilliance. These are just a few more bullets on the growing list of benefits of vibration training and vibration therapy. They are also just a few more reasons that we need to keep fighting to establish its legitimacy, and hopefully open everyone’s eyes, and brains, to its incomparable value. Therapeutic Vibes: A Physio?s Experiences With Vibration Therapy

After approximately 18 months of reading, contemplating and theorizing, over 6 months of tests and trials on over 100 willing participants in the form of physical therapy patients, and one inspirational “debate” on this site, I determined that it was time for me to sit down and organize my thoughts and experiences thus far with vibration therapy.

I am presenting these thoughts and experiences to you, fellow vibration enthusiasts; thoughts and experiences brought about through careful analysis and cautious experimentation. My intention here is to do nothing more than provoke thought and encourage further discussion on the present and future applications of vibration THERAPY.

For the “research obsessed” readers, before the smoke billows out of your ears, note that I do fully realize that most of what I am presenting here is not indisputably backed by research nor am I suggesting that anyone else utilize my methods without carefully considering whether these methods are appropriate for their individual clinical situations. In the words of fellow forum visitor Larry Leigh,

“In the end result, even though health professionals preach ‘evidence-based practice’, they all use methods and techniques which WORK FOR THEM. They cannot always produce 10 research studies which back up that particular technique.”

I am aware that my usage of a whole body vibration platform for therapy may be considered too experimental by some, but it is not without thought, experience, and consideration of well established scientific principles. In the words of the Seneca Indian tribe, “It’s not because things are difficult that we dare not venture. It’s because we dare not venture that they are difficult.” The only way to continue to evolve the science and legitimacy of these amazing devices is to continuously ask yourself, “what if?”.
Origins

As a Physical Therapist, I spend a great deal of time “manipulating” my patient’s bodies. Through successful “manipulation” of their musculoskeletal system, lymphatic system, circulatory system, and nervous system, I am able to reduce pain, encourage tissue healing, restore efficient movement, strengthen their bodies, and promote long term health and wellness.

Throughout my 10 years working with patients it has been the nervous system that has proven to be the most challenging of all to manipulate and it is because of this difficulty that I became interested in whole body vibration (WBV) and the concept of vibration therapy.

Initially, I only viewed WBV as a select means to utilize the nervous system, via the stretch reflex, to rapidly strengthen my patient’s weak muscles and in doing so, improve their motor function. Having done little research at the time, I did not realize how many other potential benefits it could potentially provide to my patients; benefits that I will discuss in more detail later. Finally, after reading through as much of the research and information as possible, spending hours browsing through posts on this site, trying out several platforms, and speaking with several WBV manufacturer representatives, I chose a Hypergravity platform (not a plug…I swear!), typed up my WBV waiver, and prepared to revolutionize my practice.

Well, it didn’t quite go as planned! When it came time to use it with my patients initially, I found myself questioning whether a majority of my patients were appropriate candidates for vibration therapy. What was the reason behind this hesitation you ask? They all shared a common feature….they were in PAIN! I had become so consumed with the idea of strengthening that I failed to realize that most people at the strengthening phase of their therapy program were already used to other forms of exercise and were therefore less “pliable” to newer, unfamiliar forms of strengthening.

It was the patients in the early phase of treatment that needed to be introduced to vibration therapy and I couldn’t quite figure out how to do that without potentially flaring them up and forcing them to “run for the hills”. My inexperience, fears of malpractice suits, and the difficulty finding reliable information on vibration therapy began to quickly overpower my drive to provide a new, unique, and potentially beneficial form of treatment to my patients. Fortunately, one of my patients, a pain management researcher at Sloan Kettering Hospital in New York, reintroduced me to the Gate Control Theory.

By applying this theory, I realized that the vibratory feedback of the machine itself could be the key to not only introducing my patients to vibration therapy gently, but to creating immediate reductions in their pain levels therefore allowing them to further progress and eventually experience the many other benefits of vibration therapy. The benefits presented so clearly in the previous articles on this site. After success with several patients, I developed a vibration therapy progression that has, thus far, proven quite helpful in efficiently and thoroughly rehabilitating my patients; no matter what their needs be.

Introduction to Vibration Training and Vibration Therapy:

As a Physical Therapist and fitness enthusiast, I have had the distinct pleasure of experiencing both firsthand, and secondhand through my patients, the sense of euphoria, clarity, and enhanced vivaciousness that comes as a result of utilizing a Whole Body Vibration platform for training or therapeutic purposes (now referred to in the industry as “Vibration Training (VTr)” and “Vibration Therapy (VTh)”). Not to be confused with deleterious, industrial and occupational whole body vibration, this form of active exercise has already demonstrated its enormous potential to improve bone density (#1), balance responses (#2), arterial circulation (#3), and muscle strength (#4). These benefits have been described in numerous studies conducted throughout the world for the past 15+ years. Recently however, as interest from the western medicine world has increased, we have seen new and exciting potential for this technology, including the treatment of neurological diseases. In a research study just released by the University of Miami, researchers demonstrated that after a one month Whole Body Vibration therapy program, subjects with incomplete spinal cord injuries were able to increase walking speed, cadence, step length and consistency of intra-limb coordination (#5). It is extremely clear that the need now exists to look beyond the categorical musculoskeletal effects and explore the more intricate and potentially more profound role of VTr and VTh on positively influencing the vast functions of the central nervous system. Through a better understanding of these neurophysiological mechanisms, this technology’s existing applications will be better clarified while allowing for the further development of its potential relevance in areas such as pain management, neuroendocrine function, mood alteration, memory, and the improvement of the efficiency and strength of the neurophysiological pathways that exist throughout the human body. The purpose of this particular article is to explore the role of VTr and VTh in helping promote increased serotonergic activity, neurogenesis via expression of Brain Derived Neurotrophic Factor (BDNF), and the associated, rapid release of enkephalins and endorphins into the bloodstream.

 

 

 

The Tonic Stretch Reflex and Serotonin:

To begin this exploration, it is important to first define VTr and VTh as well as the primary mechanisms by which this form of exercise interacts with the human body. Dangerous, industrial and occupational whole body vibration typically refers to low frequency, high amplitude industrial vibration being delivered to a body in an anatomically undesirable and physically inefficient position (for example, a jackhammer operator or semi truck driver). This differs from VTr and VTh in that these methods generally utilize higher frequency, lower amplitude vibration being delivered in a linear (straight up and down) or pivotal (teeter-totter) direction to an individual in a supervised, anatomically desirable exercise position (squat, lunge or push up, plank etc.).  

The primary mechanism by which VTr and VTh interacts with the human body is through the Tonic Stretch Reflex (TSR). This reflex occurs in response to a bombardment of the CNS provided by repeated muscle spindle excitation. The result is what is considered to be a more complicated version of the monosynaptic stretch reflex. In the case of the TSR, the reflex would include polysynaptic pathways and the need for modulation by specific descending pathways that control posture and muscle tone (#6).  Although many pathways may exert an influence over this modulation activity, the primary pathways are the vestibulospinal and reticulospinal tracts.

Of more importance when considering the purpose of this article however, we must turn our attention to the reticulospinal tract. An extrapyramidal tract, the reticulospinal tract originates in the reticular formation. The reticular formation, according to Basant et al. is a loose network of neurons located in the brainstem and containing projections to many other regions of the brain via the raphe nuclei. Through the excitatory mechanisms provided by the TSR, through exposure to a whole body vibration platform, the raphe nuclei, which are primary stores of serotonin in the brain, are activated. This activation results in increased serotonergic pathway activity and the release of serotonin. Serotonin, a type of neurotransmitter, is responsible for mood elevation, feelings of ecstasy, increased sexual desire and function, increased motivation, and improved sleep. This concept is supported by investigations involving electrical stimulation of the raphe nuclei located within the reticular formation (#7).

 

 

 

Clinical Applications – Fibromyalgia:

Throughout the medical community, low serotonin levels have been linked to many different conditions including depression, weight gain, sleep disorders, migraine headaches, and restless leg syndrome. In the physical therapy clinic however, the largest group that may benefit from increased serotonin production are those that suffer from Fibromyalgia (FM). Recent studies have shown that a common characteristic between Fibromyalgia patients is low serotonin levels. As a result, studies with medications designed to reduce the reuptake of serotonin in the brain have provided a valuable insight into the potential causes and treatment of FM (#8). Additionally, it has been established that lower serotonin levels signals the release of substance P. Substance P, another neurotransmitter, is responsible for transmitting pain signals throughout the central nervous system and thought to be responsible for the increased pain sensitivity seen in FM. This diagnosis, worthy of entire article in itself, is an increasingly common diagnosis seen throughout the rehabilitation world. Once thought of as a “wastebasket diagnosis”, the reality of this condition is quite apparent and the potential to utilize Vth and Vtr as a treatment modality demands further investigation.

Serotonin, BDNF and Neurogenesis:

The reciprocal relationship between serotonergic activity and the expression of BDNF has been well established (#9). As serotonin levels rise in the brain, BDNF expression is encouraged. Reciprocally, as BDNF expression in the brain is increased, serotonergic activity and serotonin release is supported. If you are unfamiliar with BDNF, now is the time to familiarize yourself. BDNF is a neurotrophin which acts upon neurons of both the central nervous system and the peripheral nervous system. Related to Nerve Growth Factor, BDNF’s role is to encourage the growth and differentiation of new neurons, while as the same time, protect existing neurons and increase the efficiency of communication between them (referred to as synaptic plasticity). This process is referred to as neurogenesis.

Until only recently, the idea that the brain matured at age 25 was considered factual. This was further enforced by the concept that nerve cells were incapable of dividing or replicating. These “facts” are now being challenged as recent studies have shown that the adult brain is capable of generating new brain cells (#10). This finding is the primary foundation behind the increasingly promising yet most intensely debated concept of neuroplasticity. Neurogenesis occurs in the hippocampus. Part of the limbic system, the hippocampus is where higher learning and memory occurs and has been associated with Alzheimer’s disease, epilepsy and post-traumatic stress disorder. The potential applications of promoting neurogenesis to occur in the adult brain are endless and although this concept is fairly new, any potential treatment modality, like above in the case of FM, must be considered and explored.

The Tonic Stretch Reflex and Enkephalins:

Returning to the influence of the TSR on the raphe nuclei, we must finally consider that besides initiating serotonergic activity there is also an association with enkephalin release through the interaction with enkephalin interneurons in the spinal cord (#11). Enkephalins, sometimes used interchangeably with the term endorphins, are naturally occurring opioids in the human body. When released, they are responsible for blocking pain and reducing nociceptor activity by binding to opioid receptors in the brain. The association between exercise and enkephalin and endorphin release is what has been referred to as a “Runner’s High”. This arguably addictive phenomenon has been the subject of debate for many years. However, recent research completed in Germany using positive emission topography (#12) has confirmed the very real existence of this phenomenon. Besides the potential applications for pain management with the increased production of enkephalins, the ability to create an “addiction to exercise” holds tremendous potential for fighting the war against obesity; something that researchers in the medical, health and fitness fields have spent tireless efforts to create through alternative means.

 

The Future:

Like quantum physics and the activities of subatomic particles, our understanding of the physiological mechanisms by which the CNS works has improved significantly in recent years. As it has improved however, like subatomic particles, the potential for these physiological mechanisms to occur differently than we have understood in the past has also proven to be much greater. The above pathways through which Whole Body Vibration may interact with the human body have been demonstrated in clinical research. A clinical demonstration of how these pathways occur in response specifically to VTr and Vth is still needed. The applications of this technology are far reaching and as we continue to pursue the refinement and clinical significance of Whole Body Vibration, we will hopefully be able to break through the barriers that still exist in the field of rehabilitation and move on to the next stage of fighting disease, improving quality of life for our patients, and encouraging the never ending continual pursuit of eliminating disability, reducing health care costs and building stronger, more efficient “mind-body” neural connectivity.
